Milford Primary Care Associates Office Policies

New Patients

Milford Primary Care Associates welcomes you as a new patient to our practice. Your records from your current Primary Care provider will need to be transferred to our office prior to your first appointment. Please contact our dedicated Medical Receptionists team at 302-536-2580 for more information.

What to bring to your appointments?

As your primary care provider, we want to assure that your visit is as seamless as possible. Please arrive at least 15 minutes before your scheduled appointment time.
We will need you to bring the following items to your appointments.

  • Your current insurance card (If you have more than one insurance card, please bring all of them to your appointment.)
  • A valid photo ID
  • Your Co-payment, if your insurance requires you to pay one at the time of service. Please check the front of your insurance card for the amount.
  • List of all your current medications and dosage of each medication.

After Hours Emergencies

Please call us first! We have same-day and next-day appointments available. For our established Milford Primary Care Associates patients, an on-call physician is available 24/7.

If you have an urgent medical problem that cannot wait until the office opens the next business day at 8:00 AM, please call us at 302-536-2580, and your call will be routed to the physician on-call.

If you have a true medical emergency, please call 911.

Financial Policy

Insurance Coverage

Milford Primary Care Associates will file insurance claims on your behalf for insurances that we participate. It is your responsibility to know the benefits covered by your insurance policy. This includes your co-pays, co-insurance(s) deductibles, and non-covered services. Any amount not covered by your insurance company is your financial responsibility. If you have any questions regarding your insurance coverage, please contact your insurance carrier directly. Their member services contact number can be found on your insurance card.

Method of Payments

All copays and account balances are due at the time of service. We accept the following methods for payment:

  • Bank Debit Card
  • MasterCard
  • Visa
  • Cash

Missed Appointments
Our office requires 24-hour notice if you are canceling your appointment. If you do not show for your appointment and our office was not contacted within 24 hours you will be charged a no-show fee of $50.00 effective on November 1, 2022.

Motor Vehicle Claims

All Motor Vehicle appointments will need the following information presented prior to being seen:

  • Motor Vehicle Insurance Name
  • Claim Number
  • Insurance Adjusters Name
  • Insurance Adjusters Phone Number
  • Mailing Address to file Insurance Claim

If the above information is not presented before being seen by the provider, patient will be responsible for paying the entire bill in full at the time of service.

Form & Letter Fees

It is the goal at Milford Primary Care Associates to accommodate as many form and letter requests as possible in an accurate and timely manner. All personal information must be completed on the form or they will not be accepted. Turnaround time for forms & letters is 7 business days. Forms will only be completed for those accounts in good standing. Any outstanding balances need to be paid in full prior to forms being completed by the provider. Please keep in mind that some forms require a current examination prior to being completed and the medical receptionist team will schedule you an appointment. All payments for forms and letters are due at the time of pickup. Our form fee policy is effective on November 1, 2022.

  • Each Form $25.00
  • Physical Forms presented during an exam, no charge
  • Physical Forms presented after an exam, $15.00 for each form
  • Medical Letters, $20 each letter
